Commercial Earthquake Insurance Submit ACORD 125 and ACORD property section. For risks with 5 or more locations, submit www.altageneral.com/page26.php in lieu of ACORD property section. Building construction should be determined using the following guide: www.altageneral.com/page23.php Submissions should include a copy of the latest earthquake inspection report. But remember each new risk will be reinspected. Provide key location information including any adverse soil conditions, tuck-under parking and the like. Please also provide a loss history along with expiring carriers and associated layering and premiums. And provide requested perils and deductibles. Send sumbissions to underwriting@altageneral.com
